Admission Procedure
  • Download the application form.
  • Fill it up thoroughly.
  • Keep the Enrolment Column Blank.
  • Attach all your relevant educational documents including Medical Council Registration either scanned or Photostat (Xerox) with the form and send to our address. Also you can mail us all your scanned copy of documents to avoid postal delay.

Documents Required
  • Filled up application form.
  • Copy of Bachelor Degree Certificate.
  • Copy of Medical Council Registration.
  • Copy of Internship Certificate. (Available, If any)
  • Copy of Post-Graduate Certificate. (Available, If any)
  • 3 Copies of Current Photo (Passport Size).
  • Demand Draft of Course Fee.
