3. Keep the Enrolment Column Blank.

4. Attach all your relevant educational documents including Medical Council Registration either scanned or Photostat with the form and send to our address. Also you can mail us all your scanned copy of documents to avoid postal delay.

2. Bachelor’s degree Certificate.

3. Medical Council Registration.

4. Internship Certificate if any obtained.

5. 3 Copy photos.

6. Demand Draft of Course Fee.

7. PG Certificate if any.
